Ladies and Gentlemen
As of 25 May 2018, Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (GDPR) (hereinafter referred to as the Regulation or GDPR) will apply. Therefore, Dortech sp. z o. o. with its registered office in Pobiedziska 62-010, at 8 Partnerska Street, hereby informs that as of 25 May 2018, you will have the rights specified below related to the processing of your personal data by Dortech sp. z o.o.
Pursuant to Article 13 paragraphs 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (GDPR), we inform you that:
1) the controller in accordance with Article 4, point 7 of the GDPR is Dortech sp. z o. o., ul. Partnerska 8, 62-010 Pobiedziska
2) Your data is processed for the purpose of implementing the concluded contract.
3) the recipients of your personal data are entities authorized by law, such as the Social Insurance Institution, the Tax Office, the National Labor Inspectorate, enforcement authorities, courts, the National Court Register, insurance companies, as well as entities and IT companies cooperating with the Controller;
4) Your personal data will not be transferred to a third country or an international organization;
5) Your personal data will be stored for a period of
- in connection with the conclusion of the contract, for the purpose of its implementation – for the duration of the contract,
- in the scope of establishing and pursuing own claims or defending against submitted claims – until the statute of limitations for potential claims arising from the contract or otherwise expires,
- in the scope of fulfilling the legal obligations incumbent on the Controller – for the period in which the provisions of law require the Controller to store the documentation and fulfill the obligations arising therefrom towards you,
6) You are entitled to:
a) the right to access the content of the data, pursuant to Article 15 of the Regulation;
b) the right to rectification of data, pursuant to Article 16 of the Regulation;
c) the right to delete data, pursuant to Article 17 of the Regulation;
d) the right to restrict data processing, pursuant to Article 18 of the Regulation;
e) the right to object to data processing, pursuant to Article 21 of the Regulation;
f) the right to data portability, pursuant to Article 20 of the Regulation;
7) you have the right to withdraw consent at any time without affecting the lawfulness of processing based on consent before its withdrawal, if the processing is based on previously issued consent to processing pursuant to Article 6(1)(a) or Article 9(2)(a) of the GDPR;
8) you have the right to lodge a complaint with the supervisory authority if you believe that the processing of your personal data violates the GDPR;
9) providing your personal data is a necessary condition for the provision of services and the consequence of not providing data will be the inability to provide services (performance of the contract);
10) Your data will NOT be subject to automated decision-making, including profiling, referred to in Article 22 paragraphs 1 and 4 of the GDPR.
